Herbal Remedies for Chronic Fatigue: What the Evidence Actually Shows

Chronic fatigue syndrome (ME/CFS) is a serious illness that requires professional diagnosis and management.

If you’ve been told your exhaustion is just stress, or that you need to exercise more, you probably know how dismissive that feels. Chronic fatigue isn’t ordinary tiredness — it doesn’t lift after a good night’s sleep, it doesn’t respond to a weekend off, and it often gets worse when you push through it. That last part is called post-exertional malaise, and it’s one of the defining features of ME/CFS (myalgic encephalomyelitis/chronic fatigue syndrome).

Conventional medicine doesn’t yet have a reliable treatment. There’s no FDA-approved drug for ME/CFS, and the Mayo Clinic acknowledges the condition remains poorly understood. So it makes sense that many people look to herbs — particularly adaptogens — for support.

Some of those herbs have genuine clinical evidence behind them. Others are more traditional. Here’s an honest breakdown of what actually works, what the evidence says, and what to watch out for.

What Makes Chronic Fatigue Different?

Before reaching for any supplement, it’s worth understanding what you’re dealing with. ME/CFS is characterised by several core features: profound fatigue lasting six months or more, worsening of symptoms after physical or mental exertion, unrefreshing sleep, and often cognitive difficulties (sometimes called “brain fog”). Many people also experience immune system dysfunction and autonomic nervous system disturbances.

This matters for herb selection because the best-supported herbal approaches don’t just stimulate energy — they work on the underlying stress-regulatory systems that appear to malfunction in fatigue-related conditions. That’s where adaptogens come in.

The Best-Studied Herbs for Chronic Fatigue

Rhodiola rosea — The Strongest Clinical Evidence

Rhodiola is the herb with the most direct clinical research for fatigue. A 2017 open-label clinical trial found that 400 mg daily of a standardised Rhodiola extract (WS 1375) led to meaningful improvements in subjects with prolonged or chronic fatigue symptoms over eight weeks. Participants showed significant improvements in fatigue severity, quality of life, mood, and concentration.

The European Medicines Agency has formally approved Rhodiola’s traditional use for the temporary relief of stress symptoms, including fatigue and exhaustion — one of the few herbal approvals that specifically names fatigue. Its mechanism involves modulating the body’s stress-response system, partly through effects on the heat-shock protein Hsp70 and cortisol regulation.

A systematic review in the journal Phytomedicine concluded that Rhodiola shows consistent benefit for both physical and mental fatigue, though it noted that more rigorous large-scale trials are needed before firm conclusions can be drawn.

Standard dose: 200–400 mg daily of a standardised extract (minimum 3% rosavins, 1% salidroside). Take in the morning or early afternoon — Rhodiola has mild stimulant properties and can disrupt sleep if taken late in the day.
Onset: Most people notice a difference within two to four weeks of consistent use.
Form: Capsule or tablet of standardised root extract. Avoid non-standardised powders where potency is unpredictable.

Ashwagandha — For Fatigue Driven by Stress and Poor Sleep

Ashwagandha (Withania somnifera) works differently from Rhodiola. While Rhodiola tends to have a more activating effect, ashwagandha is calming — it lowers cortisol, supports sleep quality, and helps the body regulate its response to chronic stress. For fatigue that stems from burnout, long-term stress, or disrupted sleep, this distinction matters.

A 2023 randomised, double-blind, placebo-controlled trial evaluated a standardised ashwagandha extract (Witholytin) at 200 mg twice daily in adults experiencing high stress and fatigue. After 12 weeks, participants showed significant reductions in fatigue scores, anxiety, and cortisol levels compared to placebo.

The NIH’s Office of Dietary Supplements acknowledges that ashwagandha may help reduce stress and improve sleep quality, though it notes the evidence base is still growing. The most studied forms are KSM-66 and Sensoril — both are standardised root extracts used in the majority of published trials.

Standard dose: 300–600 mg daily of standardised root extract (KSM-66 or Sensoril). Can be taken in one dose or split morning/evening.
Timing: Evening dosing suits people who also struggle with sleep; morning for those primarily dealing with daytime fatigue.
Onset: 4–8 weeks for meaningful fatigue benefit. Some improvements in sleep may appear sooner.
Maximum: Do not exceed 600 mg/day without guidance from a practitioner.

Panax Ginseng — For Physical Fatigue and Low Energy

Korean red ginseng (Panax ginseng) has one of the longest documented histories as an energy tonic, and the clinical evidence is reasonably solid. A randomised, double-blind, placebo-controlled trial in the Journal of the Korean Medical Science found significant anti-fatigue effects in patients with idiopathic chronic fatigue, with improvements in antioxidant enzyme activity suggesting a mechanism beyond simple stimulation.

A 2022 systematic review examined multiple studies on ginseng for chronic fatigue syndrome specifically and found positive signals, though it cautioned that larger, more rigorous trials are needed. Ginsenosides — the active compounds — appear to modulate immune function, reduce oxidative stress, and support mitochondrial energy production.

Ginseng is best suited for people whose main complaint is physical energy and endurance. It tends to be more stimulating than ashwagandha, so it’s worth starting at a lower dose to gauge your response.

Standard dose: 200–400 mg daily of standardised extract (containing a minimum of 4–7% ginsenosides). Take in the morning.
Form: Capsule or tablet of standardised Korean red ginseng. Avoid cheap “ginseng” teas or supplements that don’t disclose ginsenoside content.
Onset: 4–6 weeks.
Cycle: Traditional practice recommends taking breaks — use for 8–12 weeks, rest for 2–4 weeks before resuming.

Astragalus — For Immune Support Alongside Fatigue

Astragalus (Astragalus membranaceus) takes a different approach — it targets the immune dysfunction that often accompanies chronic fatigue rather than directly addressing energy levels. It’s been used in traditional Chinese medicine as a foundational tonic for centuries, and modern research has begun to validate some of those uses.

A 2023 review of 19 studies involving over 1,000 participants found that astragalus enhanced immune responses and reduced pro-inflammatory cytokines. Animal studies have also shown it can reduce markers of physical fatigue, including serum lactate and ammonia levels. Human clinical evidence specifically for ME/CFS remains limited, but astragalus is frequently combined with other adaptogens in formulas that have been studied for fatigue outcomes.

The Memorial Sloan Kettering Cancer Center herb database notes that astragalus has a good safety profile at standard doses and is one of the better-tolerated long-term tonics in herbal medicine.

Standard dose: 1,000–2,000 mg daily of dried root extract, or 500–1,000 mg of a 4:1 extract.
Timing: Can be taken at any time of day, with or without food.
Onset: Immune-modulating effects tend to emerge over 6–12 weeks of consistent use. This is a slow-builder, not an acute remedy.

How to Use These Herbs Practically

Most people start with one adaptogen rather than several at once — this makes it easier to gauge what’s actually helping. If your fatigue is primarily stress-related and affects sleep, ashwagandha is the logical starting point. If physical energy and endurance are the main issues, Rhodiola or ginseng is a better fit. Astragalus can be added once a baseline is established, particularly if you’re also dealing with frequent infections or immune vulnerability.

Capsules and tablets of standardised extracts are the most reliable form — they allow you to know exactly what dose you’re taking. Teas and loose herbs can be pleasant, but potency varies significantly between batches.

None of these herbs are fast fixes. Four weeks is the minimum time to assess any adaptogen. Give any single herb eight to twelve weeks before drawing conclusions, and keep a simple fatigue log so you have actual data rather than a vague impression.

Drug Interactions and What to Avoid

These herbs are not risk-free, especially when combined with medications:

  • Rhodiola rosea: May interact with antidepressants, particularly SSRIs and MAOIs, due to its effects on serotonin and dopamine. Avoid combining without medical supervision. May also interact with diabetes medications and anticoagulants.
  • Ashwagandha: Can interact with thyroid medications (it may raise T3/T4 levels), immunosuppressants, and sedatives. People with autoimmune conditions should use with caution — ashwagandha stimulates immune function and may worsen conditions like lupus or rheumatoid arthritis.
  • Panax ginseng: Interacts with warfarin and other anticoagulants (may reduce their effectiveness). Can raise blood pressure at higher doses. Avoid combining with stimulants including caffeine in large amounts. May affect blood sugar levels in people on diabetes medication.
  • Astragalus: May reduce the effectiveness of immunosuppressant medications including cyclosporine. Because it stimulates immune function, it’s contraindicated in autoimmune conditions and organ transplant patients.

Always disclose herbal supplement use to your GP or specialist, particularly if you’re on any long-term prescription medication.

Who Should Avoid These Herbs

Pregnancy and breastfeeding: None of the adaptogens above have been adequately studied in pregnancy. Ashwagandha in particular has traditional use as a uterine stimulant at high doses — avoid during pregnancy. Ginseng should also be avoided in pregnancy.

Children and adolescents: These herbs haven’t been studied in children or teenagers with CFS. Do not use without paediatric specialist guidance.

Autoimmune conditions: Astragalus and ashwagandha both stimulate immune function, which can worsen autoimmune conditions. Seek specialist advice before using either.

Pre-surgery: Discontinue all adaptogens at least two weeks before any surgical procedure, as several affect blood pressure, coagulation, and anaesthetic response.

When to See a Doctor

Herbal remedies can play a supporting role in managing chronic fatigue — but they are not a replacement for proper diagnosis and professional oversight. See a doctor if:

  • Your fatigue has persisted for more than six weeks without a clear cause
  • You experience significant cognitive symptoms (brain fog, memory difficulties)
  • Symptoms worsen after physical or mental activity (post-exertional malaise is a red flag that needs evaluation)
  • You have unexplained weight loss, night sweats, or swollen lymph nodes
  • Fatigue is severely affecting your ability to work or carry out daily activities

ME/CFS can mimic — or overlap with — thyroid disorders, anaemia, sleep apnoea, depression, and other conditions that have specific treatments. Ruling those out is the first priority, not the last.

If you do have a confirmed CFS diagnosis, adaptogens are best used as part of a wider management plan developed with a practitioner who understands the condition. The most important principle in ME/CFS management is pacing — working within your energy envelope rather than pushing through fatigue. No herb changes that.
